Eyelid Surgery

There is no area of the face that is more likely to show the signs of aging than the eyes. Due to a variety of factors including heredity, environment and age, the upper and lower eyelids are prone to the development of crepey skin, discoloration, and puffiness. Eyelid rejuvenation has undergone a number of evolutionary changes over the past decade, with the incorporation of nonsurgical, minimally invasive, and "minimal touch" techniques.

Dr. Kolker is an excellent surgeon! He performed my eyelid surgery and enhanced my upper lip area. The results are extraordinary and I continue to use Dr. Kolker for all of my surgical and nonsurgical needs. There is no other Doctor that I would use. H.R. / New Jersey

An eyelid lift is aesthetic plastic surgery of the eyelids that can address all facets of aging and the tired look that often develops around the eyes. Skilled in all nuances and techniques in rejuvenation of the eye area and blepharoplasty, NYC plastic surgeon Adam R. Kolker, M.D., brings vast experience, technical finesse, meticulous attention to detail, and artistry to achieve the most beautiful results.

A traditional blepharoplasty is an eyelid lift that might include any or all of the following: removal of excess upper eyelid skin, removal of excess eyelid fat, and removal of excess lower eyelid skin. To these basic cornerstones of blepharoplasty, a variety of improvements, additions and modifications have been added, and are currently available for eyelid rejuvenation. The eyelid rejuvenation techniques that may be used alone or in combination include:

  • Nonsurgical lower eyelid rejuvenation - restoring volume to the tear trough and softening the lid-cheek junction with injectable facial fillers
  • Laser resurfacing - using laser treatments to improve color, texture, and tone of the eyelid area
  • Skin-pinch blepharoplasty - removing a small amount of excess skin of the lower eyelid with an inconspicuous incision beneath the lash line
  • Transconjunctival blepharoplasty - the removal of excess lower eyelid fat without any external incisions
  • Fat grafts for tear trough and volume loss - autologous fat, harvested from your own body, is transferred to restore youthfulness to the eyelid area
  • Lower eyelid fat repositioning - tightening the lower lid support mechanisms which reduces puffiness and increases and maintains youthfulness without removing fat

Blepharoplasty with Dr. Adam Kolker

The eyes may be the facial features that best identify an individual, and maintaining the natural shape and "personality" of the eyes is of paramount importance. It is always with this goal in mind that any treatments around the eyes are undertaken. Subtle changes that rejuvenate and beautify are always preferred.

The eyelid lift approach must be precisely personalized for every nuance of skin, fat, support, shape, function and individual desire. Just as each individual's physical attributes and desires are different, so too is every surgeon unique, and the plastic surgeon you choose for eyelid rejuvenation makes a difference. What differentiates the care Dr. Kolker and his team provide is comprehensive:

  • The appreciation of the importance of maintaining natural eyelid shape and enhancing youthfulness and beauty,
  • Exceptional finesse, skill, experience, and artistry,
  • The most rigorous certification, accreditation, and safety standards,
  • Passion for achieving gorgeous blepharoplasty and eyelid rejuvenation results that can be both dramatic and subtle,
  • A connection between patient, plastic surgeon, nurse and office staff that is extraordinarily warm, intimate, and comfortable.
  • From your first visit, through the eyelid lift perioperative process, and throughout the entirety of your recovery and follow-up, you will experience the thoroughness, attention to detail, and warmth with which Dr. Kolker and his team welcome and care for you.

Dr. Kolker's Message to Those Considering Eyelid Lift

The eyes are perhaps the most uniquely identifying facial feature, and maintaining the natural, beautiful identity of the eyes is essential. The best blepharoplasty technique must be uniquely tailored to the individual. You should never look different after eyelid lift, rather, you should just look more youthful, refreshed, and rejuvenated. Dr. Adam Kolker

Planning Your Blepharoplasty

The blepharoplasty planning process begins with a comprehensive discussion and examination with Dr. Kolker in his office on Park Avenue in New York. Eyelid surgery may indeed be recommended, however there are a variety of situations in which Dr. Kolker will recommend a nonsurgical eyelid rejuvenation approach. When plastic surgery is required, the most minimal approach is selected to minimize downtime and to achieve your goals. When appropriate, plastic surgery of the eyelids is combined with other modalities, such as laser, fat grafting and fillers, to promote the most natural results.

Is there a minimally invasive, nonsurgical eyelid lift? Injectable fillers, such as Juvederm or Restylane, can "refill" the tear trough and areas of volume loss, Botox Cosmetic can "relax" and soften the fine lines and wrinkles of the crow's feet region, and laser (ablative with CO2 laser, or nonablative with Fraxel laser) can "resurface" the eyelid region. While blepharoplasty may be required for more prominent issues including excess skin and more pronounced fat bulge, for select individuals with more modest concerns, these nonsurgical maneuvers may be an outstanding solution.

Preparing for Eyelid Surgery

Approximately two weeks prior to blepharoplasty, you will have another visit with us in the office. A complete list of "dos and don'ts" is reviewed. Prescriptions for medications to be taken after surgery are given, and the pre and postop instructions are reviewed. Routine laboratory testing is ordered. Any individuals with pre-existing ocular issues or who are prone to dry-eye should see their ophthalmologist prior to the procedure.

The Eyelid Surgery Procedure

Performed entirely by Dr. Kolker in his fully accredited surgical facility on Park Avenue in NYC, blepharoplasty is an outpatient procedure. The office-based surgery suite is accredited by the American Association for the Accreditation of Ambulatory Surgery Facilities (AAAASF). An upper lid blepharoplasty may be performed under local anesthesia only, although light sedation may be elected. Skin-pinch blepharoplasty, or the removal of a small amount of skin from the lower lid, can also be performed under local anesthesia, however many select sedation for this procedure as well. For lower lid blepharoplasty with more involved muscle, septum, and fat maneuvers (repositioning or removal), sedation is required. When necessary, anesthesia is administered by a board-certified anesthesiologist from the Mount Sinai Medical Center. The procedure duration may range from approximately 45 minutes to two hours, but may be longer or shorter based on the specific techniques planned, anatomic requirements, and goals. The options will vary depending on your unique needs, but may include any individual or combination of the following:

  • Upper Lid Blepharoplasty - removal of excess skin, with or without fat, from the upper eyelid
  • Lower Lid Blepharoplasty - removal of excess skin from the lower eyelid, with varying maneuvers to control fat herniation
    • Transconjunctival Approach
    • Skin Pinch
    • Fat Transposition
    • Canthopexy - support of a slightly lax lower eyelid
    • Muscle and Septal Cinch (Tightening of support to reduce puffiness)
  • Fat Grafting to tear trough, midface, or cheek
  • Laser Resurfacing
    • Ablative Laser (CO2 Laser)
    • Nonablative Laser (Fraxel Laser)

Following your blepharoplasty, you will recover in our recovery room and will be cared for by our registered nurse. You will go home with a friend or relative when you are fully awake and comfortable, generally one to two hours after the completion of surgery. Cold compresses are used to minimize bruising and swelling around the eyes. The primary swelling and bruising resolves in about one week, and most go back to work or resume social activities around this time. Recovery after more limited blepharoplasty maneuvers may be only several days.

Exercise After Blepharoplasty

Dr. Kolker suggests light cardio exercises for approximately two weeks after eyelid lift, after which unrestricted exercise and activity may be resumed.

Additional Procedures to Consider with Eyelid Lift

Blepharoplasty is frequently an isolated plastic surgery procedure, however it is occasionally performed in conjunction with other facial rejuvenation procedures including fat grafting, laser resurfacing, browlift, and facelift.

Dr. Kolker will see you in follow-up periodically following eyelid lift. Suture removal is generally at 5 to 7 days after blepharoplasty. Eye drops may be recommended for several weeks. Dr. Kolker and his staff will always be available to assist you with any questions or recovery-related matters following the procedure.
