The Art of Breast Reduction Surgery

Making breasts smaller is only one of the many facets of breast reduction surgery. So much more than simply reducing breast size, the true goals of breast reduction are beautiful breasts, achieved through enhancement of breast shape, position, projection, proportion and decreased volume. The improvement of breast aesthetics, physical comfort, and self-confidence can be dramatic.

For many years I was embarrassed by my large breasts. It affected my self-esteem deeply. I wanted to have a breast reduction but my fears kept me from doing it. I am so grateful my gynecologist listened to my cry for help and referred me to Dr. Kolker. It was the best thing that could have ever happened to me.D.S. / NYC

Enlarged breasts, or breast hypertrophy, may be developmental in nature, having been present from an early age. Breast enlargement may also occur as a result of pregnancy or weight gain. Often, there will be a low-lying position of the breasts with droop or sag, as well as a nipple areolar enlargement. There may be physical manifestations of large breasts that may include neck and back pain, shoulder grooving from bra straps, and irritation beneath the breasts, particularly in warmer weather.

Whereas diet and exercise can have some impact on breast size, for most, breast reduction is the best means of achieving their goals. While some women may be pleased with the form and balance of their breasts and only desire a diminution in mass, most desire more lift, projection, perkiness, smaller and proportional areolas, better posture, the ability to more easily fit into clothing and bathing suits, and the confidence gained from increased breast and body harmony.

Planning Your Breast Reduction

There has been considerable evolution in breast reduction theory and technique over recent years. From decades past, where an "inverted-T" breast reduction, with an incision placed in the crease beneath the breast, was the only method to reduce breast size, you may now be a candidate for a diverse array of breast reduction approaches. After a thorough evaluation and discussion with Dr. Kolker in his office in NYC, breast reduction options using the most modern and individually tailored techniques will be discussed. In every case, Dr. Kolker will recommend and select the least invasive, most nominal incision approach to achieve the most beautiful results. The top breast reduction techniques that are available are as follows:

  • Circumareolar Breast Reduction - A reduction mammoplasty performed with a "doughnut" incision made around the areola only is used
  • Circumvertical Breast Reduction - A "Lollipop" reduction mammoplasty, or short scar breast reduction, with an incision around areola and a vertical line
  • Wise-Pattern Reduction - The more classical "Inverted-T" breast reduction, with an incision around the areola, a vertical line to the inframammary crease, and a horizontal line
  • Suction-Assisted Breast Reduction - a breast reduction performed with liposuction, solely, or in conjunction with the above

There are numerous factors that must be considered in the selection of the best breast reduction technique, which include:

  • The size of the breasts
  • The degree of droop or sag (ptosis)
  • Breast shape
  • Breast Symmetry
  • The projection and size of the nipple - areolar region
  • The elasticity of the breast skin
  • Physique and body proportion
  • Individual preferences, desires and goals

By blending the specific physical requirements and individual desires, the best reduction mammoplasty methods can be selected. The specific techniques and incision patterns are ultimately formulated on the anticipated final balance between the breast "foundation", consisting of breast gland and soft tissue, and the elastic properties and size of the "skin bra". Only with the greatest respect for this relationship can the most beautiful and natural breast shape be achieved, and the top breast reduction results be obtained.

Preparing for Breast Reduction

In addition to a careful physical assessment, one of the most crucial factors in the breast reduction process is ascertaining your goals. Dr. Kolker will listen carefully to your desires and preferences during your initial visit. You will return for another visit to the office approximately two weeks prior to your procedure, at which you will once again discuss your goals. Instructions and prescriptions are given at this visit, routine lab work is ordered, and any preoperative breast imaging studies that may be appropriate are arranged. Health and fitness should also be maximized preoperatively, and weight should be close to ideal. Body mass index, a metric that assesses body fat based on height and weight, should ideally be at or below 25, and certainly no higher than 27.5.

The Breast Reduction Procedure

In his fully accredited ambulatory surgical suite, located at 710 Park Avenue in Manhattan, NYC, breast reduction surgery is performed as an outpatient procedure. The surgical facility is accredited by AAAASF, the American Association for the Accreditation of Ambulatory Surgery Facilities. It is performed under general anesthesia, administered by a board-certified anesthesiologist from Mount Sinai Medical Center. The duration of the procedure is approximately four hours.

After your breast reduction in NYC, you will rest in our recovery room with a registered nurse to care for you until you are awake and comfortable, generally an hour or two. You will then be released with a friend or relative to take you home.

Taking off approximately seven days from work or studies is ideal, however those that have less strenuous work to return to can likely do so at four to five days. A surgical bra is worn for about one week, followed by sports bras and padded, structured bras for support. The final breast size and shape are reached after a "settling time" of about 12 weeks.

Exercise After Breast Reduction

Exercise may resume around one week (limited to low-impact at first). Elliptical, stationary bike, and treadmill (walking) are safe and can be used without restriction. More vigorous cardio, high impact, and strength training may resume around three to four weeks.

There are several scheduled follow-up visits with Dr. Kolker following your breast reduction surgery in NYC, at one week, three months, and one year following breast reduction. The doctor and his wonderful staff will be available to you at any time during your recovery.

Breast Reduction FAQs

Who is a candidate for breast reduction?

Women who have large breasts with or without droop are candidates for breast reduction.

Is breast reduction covered by insurance?

Occasionally. Significant symptoms that have failed conservative measures (orthopedic, chiropractic, physical therapy, dermatologic) must be present and well documented. Dr. Kolker does not participate with any third party insurers.

What are the main incision types?

Breast reduction incisions may be circumareolar (around the nipple area), circumvertical (lollipop), or inverted T. Short scar incision patterns are always favored, and the incision type and length are tailored on an individualized basis.

How long before I can get back to work?

Return to work after breast reduction is most often five to seven days.

How long until I can exercise?

Light cardio can be resumed at 7 days following breast reduction, and strength training at two weeks. Higher impact cardio (running) with a very supportive bra is allowed 6 weeks after breast reduction.

Are my breast reduction results permanent?

Breast reduction decreases breast size and lifts the breasts. Weight change or subsequent pregnancy may have an effect on breast size postoperatively, and gravitational effects can also have an impact on breast position over time.

How does breast reduction affect breastfeeding?

Most techniques that Dr. Kolker uses for breast reduction maintain continuity of the breast gland with the main lactiferous duct, so breastfeeding is likely possible after breast reduction.

Will I have decreased sensitivity?

Decreased sensation of the nipple or other areas of the breast or nipple are possible after breast reduction. While an uncommon outcome, this has little to do with incision patterns; the amount of breast tissue removed and the reduction technique have more influence on sensation.
